Is there a way to create form fields and also have it copy and paste down?
For example, I have a PDF document that is a merging of multiple documents, so there are multiple spots for NAME, DOB, SSN, etc.
Can I create a empty field on each page that would fill in the NAME if I typed it on say just the first page, same for DOB, and so forth?

If you have our PDF-XChange Standard or PRO products you can create a form in MS Word and convert to a PDF form using the Tool bar Icons added in MS Office - please see this previous series of question and answers.

There is no way to paste previous form fields entries (other than the conventional copy/paste).
