duplicate form fields not being copied with insert pages

When I insert pages with fields into an existing PDF, some fields are not copied. It seems to only be the fields that are duplicated on each page, as in only one of the two or three copies of that field appears. I have to manually add each field that is missing back to the page. Is there a way that I can prevent this from happening?

Thank you for the report, Could I first as for a screenshot of your "Insert pages" dialog, and a copy of the documents which you are working with that seem to cause this issue to appear? I can take a look with that information and see what might be causing this.
