Flipping horizontally or vertically does not work for comments that are straight lines, ovals, or rectangles. When multiple comments are selected, the horizontal and vertical flips will not work if even one of the selected comments is a line, oval, or rectangle. The straight lines and rectangles can be converted to polylines and polygons to make them work, but there is no other way to convert the ovals, so there is no workaround. Note that this issue is limited to comments. Horizontal and vertical flips can be performed on objects in the base content.

I can't determine whether this is due to a temporary unimplementation, a programmatic issue or limitation, or the ISO standard restriction. If possible, it would be great if the horizontal and vertical flipping of lines, ovals, and rectangles would work as well as the other comment tools.
Note that horizontal and vertical flipping seems to work with straight lines, ovals, and rectangles in PDF Annotator.

The following is a separate issue. However, since they are related to the same functionality, I will report it here together.
As mentioned above, horizontal flipping works for comments other than lines, ovals, and rectangles, i.e., polylines, polygons, pencils, etc. However, each time the command is executed, the position of the rotation handle is flipped vertically, with the rotation angle alternating between 0 and 180 degrees. On the other hand, when flipping vertically, the position of the rotation handle, i.e., the angle of rotation, remains constant. The behavior of the rotation handle when flipping horizontally seems a bit odd. Note that this issue is limited to comments. If a horizontal flip is performed on the base content object, the rotation handles will be displayed correctly.

All transformation matrices contain only 3 transformation commands - Rotate, Move and Scale.
Vertical and Horizontal flips can only be implemented using these commands.
This limits our capacity with these actions, so while we will look into the first item you mentioned, the second item, regarding the comment being rotated 180 degrees, is effectively "working as intended" because there is little else we can do to emulate the flip commands. Technically, comments are not intended to be flipped, and this "creative" workaround was our solution to the growing demand to allow that exact feature

I had completely overlooked your script. Thank you for taking the time to let us know. This is exactly what this function should be. I am amazed that so much can be implemented in JavaScript, and Mathew's script is truly amazing in every way.
I also learned that you have featured a list of scripts on your profile page. It's a must-see for all users.
PDF-XChange's implementation of javaScript is really excellent: The limitations are only because Adobe never provided access to those areas of pdf in their API and I understand that Tracker is wary of going beyond the API.
