Shell Extension causes crashes

When I have the shell extension (ver. 2.0.40.5) installed Windows explorer crashes whenever I open a folder with pdfs and the view is set to thumbnails. As soon as I installer the shell extension the problem goes away.
Error:
Explorer.EXE application error
unknown software exception (0xc00000fd) occurred in the application at location 0x0604e057

Click OK to terminate, Cancel to debug
Whichever button I click Windows explorer crashes.

Computer:
Dell Precision T5400, Xeon Quad Core E5420, 4 GB RAM, Radeon HD 4870
Windows XP Pro SP3 networked with roaming profile and folder redirects
All drivers, runtimes, .NET frameworks, etc are up-to-date
Adobe reader 8 installed for 3D pdf viewing

Did you install the Shell Extension from an earlier version, or only from the latest 40.5 build?

You can check the Shell Extension build number from Control Panel, Add/Remove programs, Support information dialogue.

It was installed from the latest build (downloaded from the Live-Update). I just installed this and have had no previous versions installed.

Done.
As I stated in the file comments area these are in a folder where I have hundreds of PDF specs, so it's difficult to pick one out and say "that one's causing this." All of the PDFs are from varying sources. Some are from Cadzation's Acroplot (which uses pdfXchange 3), some are from Bluebeam pdf editor, some are from Cutepdf, some are from whatever the manufacturer uses, some are from Adobe distiller.
