I downloaded the zip file for the Viewer. There are two item: pdfx_SE.exe and PDFXVwer.exe.
I am wondering which to use. What is the difference between the two items?

pdfx_SE.exe is a shell extension for Windows Explorer. As far as I know it has two functions:
The one you want for the viewer is the other one.
- It shows thumbnails of PDF files in Explorer
- It lets Vista search within PDF files

The Shell extensions should perform much better now than possibly when you last tried Bhikkhu - but we would like to here from any party that finds this is not the case.
Also the latest downloads now incorporate the shell extensions in the main installer as an optional component - rather than a seperate installler.
