PDF is blank (opens in Acro Reader)

I have a PDF that when it opens in Viewer just opens to blank pages. If I close its tab and then try to open any other PDF they are blank as well and the Viewer freezes up and has to be closed by Task Manager. If I close Viewer completely I can open another PDF with no issues. I'm uploading it so you can take a look and see what the issue is.

The file doesn't open for me.. It immediately crashes the Xchange Viewer.

Same here... PDF-XChanger viewer just disappears... no error message. Opens okay in Acrobat 9 Pro, Reader 8 and PDF Target.

The issue was with processing the startup java script in the document's interactive form.

The new version (42.10) works great opening the PDF, but now that it is open I've found that when I go to the dropdown boxes on page 2 and select anything in the dropdown, the Viewer crashes every time. Can someone take a look? Thanks!

I tested the form and all seems to work, though I could not see any drop down box on pg2 please confirm what drop down you are referring to.
