form not filling

Field boxes highlight when clicking into the form fields, but they will not accept keyboard input. A few check-boxes are working.

"Typewriter" input is not accepted anywhere on the document, but it works fine on other docs.

I am told others appear to be filling out this downloaded form. ?

thanks for the post and welcome to the forum.

Can you send us this form to look at? Either upload it in a zip file or post a URL where we can grab a copy?
