I have flattened a textbox comment
How do I un-flatten (resurrect) access ?
Looks like "Flatten" is permanent. (Like road kill)
Can "Flatten" be removed/hidden from context menu (and retained in MenuBar > Comments-menu)

Yes flattening an annotation changes it to an element of the base content of the page. Reversing that element back to an annotation is not possible.
If you've accidentally flattened an object - Ctrl-Z should undo the command in the current session. If you've however closed the document after flattening - the only way would be to e.g. create a new annotation and remove the flattened object.
The context menus can not be currently modified and I do not know if such an option is planned for the future or not.
