How to fill the attached PDF format - With square inputs

Hi, Can someone please help me fill the attached PDF document using Xchange. This document has single character square input fields and is extremely difficult to fill using the typewriter button. Any help or pointers will be much appreciated. Thank you for your help.

Thanks for the post and sample - as this is not a fillable form, there isn't any other way to fill this in other than using the typewriter/textbox tools. I would recommend that you print off the document, fill it out by hand, then scan it back in using the File --> New Document --> From Scanner feature.
