In the above document, it appears there's a bug with Viewer where it won't allow certain entries. For example, see the bottom of document page 7. For Spirituality, it won't take No for the answer, but enters it as Yes. And in the next question it won't take Either, but enters it as In the Hospital. Neither are a problem in Editor.

However, in Editor on page 9 under Organ Donation, "Any organ or body part" won't uncheck, even if the choice below is checked. That may be the document though, since it happens in PDFSam as well.

Thank you for the report, I am sorry to say that the Viewer has been discontinued for well over 10 years now, and is not seeing any further updates. If there are bugs present there, we cannot fix them any longer. Its successor, the Editor, should be used instead.

Now, regarding the issue you reported with the Editor, I was unable to reproduce this. It seems that the root cause is document specific as you suspect, but that we have built a workaround to it in recent releases, so this is now most likely an issue of the version you are running. Are you fully up to date and running the latest release (10.6.0.396)?

If not, please do update to the latest version, and you should find that this works as expected. If you find any other issues within the Editor specifically, please let me know and we can investigate further.

I am looking at that form - and it seems like the 'Any organ' and "only... ' are a separate check box group, and there is no script in that file when e.g. the "I do not want to donate..." option for the other radio group is pressed to clear those fields.
So it does seem to be working as expected - as the form is designed in that way and those checkboxes behaved exactly the same in other PDF software I tried with.
