Form can't be filled out

Form can't be filled out

Hello, the attached form can't be filled out with PDF-XChange Viewer 2 (Build 42.9). Only the checkboxes can be checked. Text can be entered in a form fielt but when leavint the form field the text goes away. Please help. Thanks.
