how to add check box

I am creating PDF survey sheet now.
I'd like to add check box to enter check mark according to writer's will later on my PDF .
Would you tell me how to add it on PDF-XChange Viewer if you know it please ?

Hello pdfa,

You will need Form Creation tools which are not currently available in our Viewer I am afraid.
Form creation is planned for after ver3 is released early in the new year.
