I have bought PDFXChangeViewer PRO, Version 2.0 (Build 41.4), because it offers all the functions I normally need and at is slim and protects resources.
2 problems, however, I can currently not solve:
Flatten Annotations/Comments:
Inserted text comments, lines, rectangles, etc., can not be copied and re-inserted. Ctrl+C, Ctrl+V do not work and are greyed.
Outlook 2003 SP 3 + Windows Vista SP 1:
If PDFXChangeViewer is the default program for editing PDF-documents, PDF documents in E-mail neither cant be saved nor cant be copied with drag + drop to harddiscs.
Under Outlook 2007 + Windows XP SP 3 it already works.
Flatten Annotations/Comments:
Inserted text comments, lines, rectangles, etc., can not be copied and re-inserted. Ctrl+C, Ctrl+V do not work and are greyed.
Copy/Paste for annotations will be added into build 42 of the viewer. By the way, it is not flatten. Flatten operation is available now (Comments -> Flatten Comments...)

Will check with Outlook 2003. Can you drag&drop from Outlook 2003 when PDF XChange Viewer isn't default application for PDF files?
